De la evidencia a la práctica clínica: una propuesta de marco clínico para la punción seca en la cefalea de tipo tensional

Antecedentes: La punción seca (PS) se utiliza con frecuencia en el manejo fisioterapéutico de la cefalea de tipo tensional (CT). Aunque estudios previos sugieren posibles beneficios en resultados relacionados con el dolor de cabeza, persiste una considerable heterogeneidad en la selección de pacientes, el músculo objetivo, la dosis del tratamiento y los procesos de toma de decisiones clínicas.
Objetivo: Proponer un marco clínicamente orientado para la aplicación de la PS en pacientes con CT, basado en la literatura científica disponible y en los patrones contemporáneos de práctica fisioterapéutica.
Métodos: Se realizaron una revisión narrativa y una síntesis conceptual, utilizando evidencia de estudios que abordan la PS, los mecanismos del dolor miofascial, la fisiopatología del dolor de cabeza y el manejo fisioterapéutico de la CT. También se consideró que los hallazgos de investigaciones recientes que examinan patrones de práctica fisioterapéutica e intervenciones en PS respaldan el desarrollo del marco.
Resultados: La evidencia disponible sugiere que la PS puede constituir un componente útil en la gestión multimodal de la fisioterapia para pacientes seleccionados con CT. Sin embargo, persiste una variabilidad considerable entre los protocolos de intervención, la selección del músculo objetivo, los parámetros de dosificación y las estrategias de implementación clínica. Con base en la síntesis narrativa de la literatura y en la evidencia de la práctica clínica, se propone un marco clínico de seis dominios, que incluye la selección de pacientes, el examen clínico, la selección del músculo objetivo, los parámetros de intervención, la integración multimodal y las estrategias de reevaluación.
Conclusiones: El marco propuesto se concibe como una herramienta conceptual para apoyar el razonamiento clínico y promover una mayor consistencia en la aplicación de la PS en la CT. No debe interpretarse como un protocolo clínico validado. La investigación futura debería evaluar su viabilidad, fiabilidad y utilidad clínica en estudios prospectivos, al tiempo que aclaran aún más los parámetros óptimos del tratamiento y los subgrupos de pacientes más propensos a beneficiarse de las intervenciones con PS.
